What income and resource limits apply to Medicaid in New York?

If you live in New York and need Medicaid, you must meet certain financial rules. These rules decide if your income and resources qualify you for coverage. Knowing these limits can help you prepare and make smart choices for your future care.

Income rules 

New York sets strict income limits for Medicaid eligibility. For an individual in 2025, the monthly income limit is about $1,800, while for couples it is around $2,433. Income includes wages, Social Security benefits, pensions, and other regular payments. If your income is higher, you may still qualify through a spend-down program, where you use excess income to cover medical costs until you reach the limit.

Resource limits for applicants

In addition to income, Medicaid looks at your available resources. For an individual, the limit is $32,396, and for couples, it is $43,781. Resources include cash, bank accounts, investments, and other assets that can be converted to cash. Some resources, like your primary home, personal belongings, and one car, do not count toward this limit. This protection allows you to keep essential property while still qualifying for Medicaid.

Spousal protections under Medicaid

When only one spouse applies for Medicaid, New York follows special spousal rules. The spouse not applying is allowed to keep a certain amount of income and resources. The community spouse may keep between $74,820 and $157,920 in resources (based on total assets) and has a monthly income allowance of $3,948.  These rules help prevent financial hardship for the spouse who does not need long-term care.
